With thanks to a new contract with DCYF, the Shepherd Program proudly announces the start of a brand new program to support families formed through adoption and foster care! The Supporting Adoptive and Foster Families Everywhere (SAFFE) Program, is an intensive, community based program aimed at preserving foster and adoptive placements for children/teens, statewide, up to age 18, that have either disclosed sexual abuse or are exhibiting symptoms of sexual abuse, such as sexualized behaviors. We are currently looking for dedicated, qualified and experienced clinicians and case managers to staff this new program! All positions are full time, salaried with benefits. SUBDIVISION: PEDIATRICS DEPARTMENT: The Medical Home Care Supervisor (LICSW) oversees the transitional support services provided to families as they transition from the NICU to the medical home with their fragile infants. Responsible for staff supervision, reporting out on program performance, coordinating care with the larger NICU team and representing the program as part of network of community partners. Required to complete clinical assessments of patient needs and coordinate care as indicated. Provides input to the Senior Division Manager on operational budget and HR compliance issues. Assists in writing grant reports, research advancement and supports the medical team as indicated. Master's degree in Social Work and a license as a Licensed Independent Clinical Social Worker (LICSW) required. Experience in case management and infants/children with special health care needs required. Proven leadership skills required. Experience in a health care setting, hospital setting preferred. Excellent communication, organization, and interpersonal skills required.
